Al Wakra begins post-discharge phone follow-up

Doha: Patients at Al Wakra Hospital (AWH) have started receiving personalised advice about managing their medications after discharge through a newly introduced Post Discharge Phone Follow-up initiative.
The initiative by the Pharmacy Department aims to ensure patients take prescribed medication correctly to further the delivery of compassionate and efficient care provided to them while on admission. 
“Under the initiative, discharged patients are provided with medication management advice via personalised phone calls. It provides pharmacists with the opportunity to reinforce discharge instructions, and identify, prevent and resolve drug- related issues such as medication compliance, adverse drug reactions, contra-indications and drug interactions,” said Dr Rasha Al Anany, Assistant Director of Pharmacy. 
“The patient will be called three days after their discharge for a review of prescribed and over-the-counter medications they are using. The pharmacist will reconcile each patient-reported medication with the medications’ list documented upon discharge. The patient will also receive individualised educational information such as medication administration, adherence and lifestyle modification in accordance to his/her prescriber.” 
The system will help in early recognition of any medication-related side effects patients may experience. 
Dr Al Anany said in case of any serious medical or disease management issues identified during calls, the pharmacist will refer patients to healthcare practitioner for further treatment.


Ramadan gifts for Aspire employees 

Doha: Aspire Zone Foundation (AZF) distributed more than 1,800 hand-crafted gifts to its staff to celebrate the start of Ramadan.
The gifts, including beautiful handmade bags filled with nuts, sweets and crackers, were created by productive families in Qatar.
